I acknowledge that if I believe any conditions are unsafe, I will immediately discontinue participation in such activities. I and the children have no illness or injury that may affect the safe enjoyment of Peekaboo Playground for us or other guests. I knowingly and freely assume full responsibility for myself and the children accompanying me to Peekaboo Playground. I freely assume all risks, both known and unknown, even if arising or resulting from negligence of Peekaboo Playground, staff of Peekaboo Playground or other customers.I for myself, my child\/children or ward\/wards, and on behalf of my or their heirs, assigns personal representatives and next of kin, hereby hold harmless Peekaboo Playground LLC and Peekaboo Playground Two LLC, Peekaboo Playground three, its owners, members, employees, equipment manufacturers, landlord and sponsoring agencies from all liability for any such personal injury, disability, death or loss of damage to person or property to the fullest extent of the law. I understand that my execution of this waiver will authorize Peekaboo Playground to enter this waiver into its database and use it as a continuous, multi use waiver for my child\/children's on going participation in any and all activities held at Peekaboo Playground until I revoke it in writing.<\/p>","created_at":"2023-05-30 15:53:07","delete":"0","expiration_date":"0","waiver_type":"Both","s3_image":null}
